1. Introduction
Welcome to Rite Now Phlebotomy & Specimen Collection. These terms govern our professional mobile phlebotomy and specimen services serving Milwaukee and surrounding Wisconsin communities. By booking an appointment, you agree to these legal conditions, which ensure safe healthcare delivery. Our team serves as a mobile lab bridge.
2. Provided Medical Services
We offer mobile blood draws, urine collection, and kit processing. All collections require a valid physician's order prior to the visit. While we provide the expert skill for the draw, we do not diagnose conditions or interpret lab results. Our responsibility ends upon successful delivery of the specimen to the designated lab partner.
3. Eligibility Rules
Patients must be under the care of a licensed physician. It is your responsibility to provide an environment that is clean, quiet, and well-lit for the procedure. You must also disclose any history of fainting or difficulty with blood draws to our staff beforehand to ensure your safety and the integrity of the collected sample.
4. Payments and Lab Fees
Our convenience fee covers travel and collection only. This is separate from any diagnostic fees charged by testing laboratories. Payment is due at the time of booking. Cancellation with less than 24 hours' notice will incur a non-refundable administrative fee. We do not currently bill insurance directly for our convenience fees.
5. Policy on Cancellations
Please value our staff's time. Cancellations must be made 24 hours in advance to avoid penalties. We allow one reschedule per booking without extra costs if notice is provided early. In instances of inclement Wisconsin weather or traffic, we will communicate any delays promptly and prioritize the safety of our technicians.
6. Liability Cap
Rite Now Phlebotomy & Specimen Collection is not liable for issues from third-party labs or medical conditions. Our liability is limited to the cost of the collection service.
