How do I book a routine mobile blood draw?
Use the Book Appointment button for a routine mobile blood draw. The booking calendar is for standard draws; provide your service location and follow the instructions for any order or kit. For a specialized request, use this inquiry form so we can review it before confirming a visit.
Is a medical requisition mandatory?
Requirements depend on the service and receiving laboratory. A valid order, requisition, or supplied collection kit may be needed. Contact us with the request so we can review what is required before scheduling.
How do I request a DNA, therapeutic, or postmortem collection?
Select the service in the inquiry form and briefly describe the collection requested. We review the applicable order or authorization, kit instructions, location, and logistics before confirming whether and when a collection can be scheduled. For urgent postmortem requests, call (414) 939-7080.
